For your convenience, we accept cash, check , Visa, MasterCard, American Express, and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Dr. Ting and Dr. Ivry are preferred providers for Delta Dental PPO insurance. Dr. Park is NOT a preferred provider for any insurance company. If you have dental benefits with other insurance companies, we will submit a claim for you so that your insurance can reimburse you directly. If you have any questions regarding your particular insurance, please contact us.
Please note, we do NOT accept HMO insurance plans.
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid it’s portion is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to any actual procedures.
Dental services are not covered by Medicare. Drs. Park, Ting and Ivry are NOT providers for Medicare. If you feel your surgery would be eligible for coverage and you desire to obtain Medicare benefits, you have the option of selecting a Medicare provide/participant at another office or facility.
