Welcome!
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally for your convenience, a surgery can be performed the same day as the consultation for healthy patients not on blood thinners. This is especially true for healthy patients scheduled to have wisdom teeth removal under general anesthesia. However, for those with a complex medical history, or those who are on blood thinners, a pre-surgical consultation will be required and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
Your surgical referral slip (given to you by your dentist or physician).
Any recent x-rays if applicable – If your dentist or physician has taken x-rays you may request that they forward them to our office or you may pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
Current list of medications
If you have medical and/or dental insurance, please submit them with your online patient registration form or bring your insurance card(s) with you for us to scan. This will allow us to help you process any insurance claims in a timely manner.
IMPORTANT: All patients under the age of 18 MUST be accompanied by a parent or legal guardian.
